vad \u00e4r det?<span class=\"ez-toc-section-end\"><\/span><\/h1>\n\n\n\n<p><strong>Hist<\/strong>amin (antik grekiska \u1f31\u03c3\u03c4\u03cc\u03c2 histos = v\u00e4vnad) \u00e4r en biogen (antik grekiska \u03b2\u03af\u03bf\u03c2 bios = liv och -genesis = uppkomst) <strong>Amin<\/strong> (organiska derivat av ammoniak (NH3)), som f\u00f6rekommer som v\u00e4vnadshormon fr\u00e4mst i huden, lungorna, magen, tarmarna och mellanhj\u00e4rnan.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Biosynthese_und_Speicherung\"><\/span>Biosyntes och lagring<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Histamin syntetiseras i mastceller, epidermis, magslemhinnan och nervceller och lagras i vesiklar som \u00e4r bundna till heparin. Det bildas fr\u00e5n aminosyran histidin genom pyridoxalfosfatberoende dekarboxylering av enzymet histidindekarboxylas i en enstegsreaktion.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Freisetzung\"><\/span>sl\u00e4ppa<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Histamin fris\u00e4tts fr\u00e5n t.ex. mastceller, basofila granulocyter, celler i luftr\u00f6ren, slemhinnorna och mag-tarmkanalen (vars ECL-celler producerar histamin fr\u00e5n gastrin, acetylkolin och PACAP (<em>hypofysens adenylatcyklasaktiverande polypeptid<\/em>)) i samband med IgE-inducerade allergiska reaktioner av omedelbar typ (typ I) eller komplementfaktorer (komplementsystemet \u00e4r en del av det ospecifika humorala (antikroppar och andra f\u00f6rsvarsrelaterade proteiner) immunsystemet).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Extern_provozierte_Freisetzung\"><\/span>Externt framkallad utl\u00f6sning<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>F\u00f6ljande l\u00e4kemedel kan bidra till en \u00f6kad fris\u00e4ttning av histamin:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Muskelavslappnande medel<\/li>\n\n\n\n<li>Opiater<\/li>\n\n\n\n<li>Plasmaexpanderare<\/li>\n\n\n\n<li>R\u00f6ntgenkontrastmedel<\/li>\n<\/ul>\n\n\n\n<p>Livsmedel inneh\u00e5ller ocks\u00e5 varierande koncentrationer av histamin.<\/p>\n\n\n\n<p>Eteriska oljor inneh\u00e5ller INTE histamin p\u00e5 grund av destillationsprocessen! <br>Av denna anledning \u00e4r det r\u00e5d som finns i media om att personer som lider av HIT b\u00f6r undvika eteriska oljor som apelsin eller citron ogrundat.<\/p>\n\n\n\n<p>Eteriska oljor erh\u00e5lls genom destillering med vatten eller etanol (som avl\u00e4gsnas efter destillering). Histamin \u00e4r l\u00f6sligt i vatten och etanol (<a href=\"https:\/\/de.wikipedia.org\/wiki\/Histamin\" target=\"_blank\" rel=\"noreferrer noopener\">k\u00e4lla<\/a>), men inte i eter, och skulle d\u00e4rf\u00f6r f\u00f6rbli i l\u00f6sning. Den eteriska oljan inneh\u00e5ller d\u00e4rf\u00f6r inte n\u00e5got histamin.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Funktionalitat\"><\/span>Funktionalitet<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Histaminets huvudsakliga funktion \u00e4r att medverka i f\u00f6rsvaret mot allergener eller fr\u00e4mmande \u00e4mnen.<br>Sett p\u00e5 cellniv\u00e5 initierar histamin aktiveringen av histaminreceptorer H<sub>1<\/sub> .. H<sub>4<\/sub>som i sin tur tillh\u00f6r de G-proteinkopplade receptorer som ansvarar f\u00f6r att \u00f6verf\u00f6ra extracellul\u00e4ra signaler in i cellen (via G-proteiner), f\u00f6rsvarsreaktionerna.<\/p>\n\n\n\n<p>Inom CNS fungerar histamin som en neurotransmittor i histaminerga neuroner. Den h\u00f6gsta koncentrationen av histamin finns i hypothalamus.<\/p>\n\n\n\n<p>S\u00f6mn- och vakenhetsrytmen styrs genom att aktivera H<sub>1<\/sub>-receptor regleras. Kr\u00e4kningar framkallas ocks\u00e5 via denna receptor. Det \u00e4r fortfarande oklart hur denna receptor \u00e4r inblandad i regleringen av aptit, blodtryck, kroppstemperatur och sm\u00e4rtk\u00e4nsla.<\/p>\n\n\n\n<p>F\u00f6rutom tarmens motilitet reglerar histamin ocks\u00e5 tarmens motilitet genom att aktivera H<sub>2<\/sub>-receptorer reglerar produktionen av magsyra.<br>Aktiveringen av H<sub>2<\/sub>-receptorer orsakar en \u00f6kning av hj\u00e4rtats slagkraft och frekvens.<\/p>\n\n\n\n<p>Den presynaptiska H<sub>3<\/sub>-receptorer p\u00e5verkar effekten av signalsubstanserna dopamin, glutamin, noradrenalin och serotonin.<\/p>\n\n\n\n<p>Histamin fungerar som en mediator vid allergier, astma, inflammation och br\u00e4nnskador och orsakar sm\u00e4rta, leder till sammandragning av glatt muskulatur, t.ex. bronker, blodk\u00e4rl (&gt; 80 \u03bcm), matsm\u00e4ltningsorgan, men \u00e4ven hud, samt kl\u00e5da och n\u00e4sselutslag. I mindre blodk\u00e4rl orsakar histamin vasodilatation, vilket resulterar i rodnad av huden.<br>Histamin leder ocks\u00e5 till fris\u00e4ttning av adrenalin, fr\u00e4mst genom aktivering av H<sub>1<\/sub>-receptorer, fr\u00e5n binjurarna.<br>Aktivering av H4-receptorn g\u00f6r att de eosinofila granulocyterna och T-lymfocyterna (tymuscellerna) larmas.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Abbau\"><\/span>Demontering<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Histamin bryts huvudsakligen ned i det centrala nervsystemet (CNS) av enzymet histamin-N-metyltransferas.<br>I annat fall sker nedbrytning fr\u00e4mst via diaminoxidas och aldehydoxidas till imidazolyl\u00e4ttiksyra, som uts\u00f6ndras via njurarna efter ribosylering.<\/p>\n\n\n\n<h1 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Histamin-Werte\"><\/span>Histaminv\u00e4rden<span class=\"ez-toc-section-end\"><\/span><\/h1>\n\n\n\n<p>Laboratoriets referensintervall f\u00f6r histamin (totalt) i leverblod (ELISA) \u00e4r &lt; 65,5 ng\/ml.<\/p>\n\n\n\n<p>10 mg\/ml leder till akut andn\u00f6d, blodtrycksfall, diarr\u00e9, kr\u00e4kningar, hudrodnad, huvudv\u00e4rk, n\u00e4sselutslag, illam\u00e5ende, medan 100 mg\/ml betraktas som f\u00f6rgiftning med motsvarande \u00f6kade symtom samt svimning, svullnad i ansiktet och tungan.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Histaminsteigernde_Komponenten\"><\/span>Histaminh\u00f6jande komponenter<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>F\u00f6rutom de ovan n\u00e4mnda l\u00e4kemedlen orsakar MAO-h\u00e4mmare (h\u00e4mmar nedbrytningen av hormonet adrenalin och signalsubstanserna, t.ex. dopamin, noradrenalin och serotonin) en \u00f6kning av histamin f\u00f6r att \u00e5stadkomma en dysreglering av hj\u00e4rnans \u00e4mnesoms\u00e4ttning vid t.ex. depression genom h\u00f6gre plasmaniv\u00e5er och koncentrationer av de aminer som n\u00e4mns i cellen. <\/p>\n\n\n\n<p>Livsmedel med histaminh\u00f6jande effekt \u00e4r t.ex: Alkohol, ananas, bananer, p\u00e4ron, b\u00f6nor, chili, jordgubbar, guava, hallon, kiwi, linser, papaya, choklad, soja, vetegroddar, citrusfrukter<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Histaminsenkende_Komponenten\"><\/span>Histamin-s\u00e4nkande komponenter<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>\u00c4pple, broccoli<strong>, <\/strong>Bl\u00e5b\u00e4r (vilda), hibiskuste, ingef\u00e4ra, kamomillte, paprika, pepparmyntste, svart kumminolja \u00e4r bland de histaminreducerande livsmedlen.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Histaminreiche_Nahrungsmittel\"><\/span>Histaminrika livsmedel<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Livsmedel som aubergine, avokado, balsamvin\u00e4ger, fisk (utom nyf\u00e5ngad eller fryst), j\u00e4stextrakt, yoghurt, kaffe, kakao, ost (ju mer mogen, desto mer), svamp, r\u00f6dvin, surk\u00e5l (inlagda livsmedel som gurkor etc.), gr\u00e4ddfil, skinka, sojas\u00e5s, spenat, te, tomater, vin\u00e4ger, citronsyra (koncentrat) h\u00f6r till de histaminrika komponenterna i kosten.Gr\u00e4ddfil, skinka, sojas\u00e5s, spenat, te, tomater, vin\u00e4ger, citronsyra (koncentrat) h\u00f6r till de histaminrika komponenterna i kosten och b\u00f6r undvikas vid HIT.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Histaminarme_Nahrungsmittel\"><\/span>Livsmedel med l\u00e5g histaminhalt<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Livsmedel som har l\u00e5g histaminhalt och d\u00e4rf\u00f6r \u00e4r s\u00e4kra f\u00f6r HIT \u00e4r t.ex. agavesirap, blomk\u00e5l, cr\u00e8me fra\u00eeche, spelt, \u00e4gg, vin\u00e4ger (\u00e4pple\/brandy), f\u00e4nk\u00e5l, f\u00e4rskost, f\u00e4rsk mj\u00f6lk, fruktjuicer, korn, gr\u00f6nk\u00e5l, gurka, UHT-mj\u00f6lk, havre, havremj\u00f6lk, hirs, honung, keso, ost (unga sorter), morot, potatis, \u00f6rtte (utom n\u00e4sslor), k\u00f6ksv\u00e4xter, pumpa, pumpafr\u00f6, gurkmeja, purjol\u00f6k, linfr\u00f6, macadamia, majs, mandelmj\u00f6lk, mandel, mangold, kastanj, mozzarella, pasta, paprika, paran\u00f6t, palsternacka, pistage, kvarg, quinoa, r\u00e4disa, rabarber, r\u00f6dbeta, r\u00f6dk\u00e5l, gr\u00e4dde, salsippa, selleri, sesam, sparris, s\u00f6tpotatis, s\u00f6tkr\u00e4mssm\u00f6r, zucchini, socker, l\u00f6k.<\/p>\n\n\n\n<p>Alla sallader har ocks\u00e5 l\u00e5g histaminhalt, med ett undantag: ruccola.<\/p>\n\n\n\n<p>Frukter som kan kategoriseras som histaminfattiga \u00e4r t.ex: \u00c4pple, aprikos, bj\u00f6rnb\u00e4r, tranb\u00e4r, dadlar, granat\u00e4pple, bl\u00e5b\u00e4r, r\u00f6da vinb\u00e4r, jostab\u00e4r, persimon, k\u00f6rsb\u00e4r, litchi, mango, melon, nektarin, persika, tranb\u00e4r, surk\u00f6rsb\u00e4r, druva<\/p>\n\n\n\n<h1 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Pathologische_Aspekte\"><\/span>Patologiska aspekter<span hj\u00e4rtklappning, hj\u00e4rtarytmi, hyperhidros, hypotension, huvudv\u00e4rk (inklusive migr\u00e4n), konjunktivit, panikattack, rinit, rinorr\u00e9, halsbr\u00e4nna, takykardi. <\/p>\n\n\n\n<p>En f\u00f6rh\u00f6jd magsyraniv\u00e5 kan tyda p\u00e5 en histamininducerad f\u00f6rsvarsreaktion.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Labormedizin_%E2%80%93_Differentialdiagnostik\"><\/span>Laboratoriemedicin - Differentialdiagnostik<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>F\u00f6rh\u00f6jda blodniv\u00e5er aktiverbarhet av mastceller (\u00f6kat antal mastceller, minskad membranstabilitet)<\/li>\n\n\n\n<li>St\u00f6rning av histaminnedbrytningen<br>- Prim\u00e4r (genetisk) brist p\u00e5 diaminoxidas (DAO) <br>  eller histamin N-metyltransferas (HNMT)<br>- Sekund\u00e4r aktivitetsbrist i DAO, utl\u00f6st av brist p\u00e5 Kuofer, vitamin B6, zink,<br>  Minskad DAO-bildning p\u00e5 grund av skadad tarmepitel eller l\u00e4kemedelsp\u00e5verkan<\/li>\n\n\n\n<li>\u00d6kad histaminbildning p\u00e5 grund av \u00f6kad kolonisering av histaminproducerande bakterier i tarmen<\/li>\n<\/ul>\n\n\n\n<p>Om en f\u00f6rh\u00f6jd histaminniv\u00e5 har p\u00e5visats enligt blodprovet b\u00f6r histaminkoncentrationen i avf\u00f6ringen best\u00e4mmas med hj\u00e4lp av ett avf\u00f6ringstest, och histaminproducerande bakterier b\u00f6r p\u00e5visas i avf\u00f6ringen f\u00f6r att klarg\u00f6ra differentialdiagnosen av orsaken.<\/p>\n\n\n\n<p>Histamin i faeces \u00e4r laboratorierefererat med &lt; 600 ng\/g (ELISA).<\/p>\n\n\n\n<p>Kulturer av histaminbildare skapas f\u00f6r detta \u00e4ndam\u00e5l.
